FAQ

Please be advised this process takes up to a period of ten (10) working days, after which a representative of the agency will contact you or your representative for collection of a formal response from the Agency in regards to the eligibility of your application.

Process Steps

  • Medical Assessment: Undergo a Barthel Medical assessment at your local clinic.
  • Score Qualification: Ensure your medical score qualifies you for home care assistance.
  • Application Submission in Person or by a Representative: Apply at the office located at Unity House, Victoria, Block A, Floor 1, Room 110.
  • Duration of Application Process: 10 working days.

Required Documents Check List

  • Medical Test Result: Original copy of Barthel Assessment, not older than 3 months.
  • Identification: ID card of the applicant and all persons residing in the home including any foreign cohabiter)
  • Proof Of Guardianship: where necessary.
  • Proof of Income: Most recent pay slips and/or documentation of benefits of applicant and for all employed persons living in the applicant’s home.
  • Bank Statements: Statements covering the last 6 months for the applicant.
  • Utility Bills: Most recent water and electricity bills.
  • Housing: Most recent rental agreement if you are currently renting and Part Rental Assistance if relevant.
  • Financial Liabilities: Loan agreements (if you have any).
  • Proof of Expenses: Documentation showing any other monthly home expenses.
  • Proof of Relationship Status: Required if Married or Divorced
  • Proof of Business: Business Registration, License Number, and Tax Return (if applicable).
